Privacy Policy

MyNeuPath collects only the information needed to schedule a consultation and provide care. This policy explains what we collect, how we use it, who we share it with, your California privacy rights, and how to contact us.

1. Information we collect

When you submit the consultation request form, we collect the information you give us:

• Full name
• Phone number
• Email address
• Preferred contact method
• The concern you select from the dropdown (this is information about your mental or physical health)
• Anything you choose to include in the optional note field

We do not use website cookies or third-party analytics that track you across other websites. Your browser may share standard request information (IP address, browser type, page requested, time of request) with our hosting provider, Vercel, in the normal course of serving the page. That information is retained by Vercel under its own privacy policy and is not used by us for marketing.

2. Sensitive Personal Information

Under the California Privacy Rights Act (Cal. Civ. Code §1798.140(ae)), information that reveals "mental or physical health condition or treatment" is Sensitive Personal Information. The concern you select on our consultation form is Sensitive Personal Information. We use it only to route your request and prepare your clinical evaluation. We do not use it to infer characteristics about you, target advertising, or sell or share it for cross-context behavioral advertising.

You have the right to limit how we use Sensitive Personal Information about you. See section 6 below.

3. How we use the information

We use the information you provide for these purposes only:

• To contact you about scheduling a consultation
• To verify insurance benefits if you ask us to
• To prepare for your clinical evaluation
• To respond to questions you raise in the optional note
• To meet legal, regulatory, and clinical-recordkeeping obligations

We do not use your information for marketing without your separate written authorization.

4. Who we share information with

We share information only with the following categories of third parties, and only as necessary to operate the clinic and deliver care:

• Healthcare providers and clinical staff who participate in your evaluation or treatment.
• Insurance carriers and payers for benefits verification, prior authorization, and claims, when you authorize us to verify your coverage.
• Service providers we use to operate the clinic — for example, the website host (Vercel), email and phone systems, electronic health record vendors, and any other Business Associates who handle Protected Health Information on our behalf. We require these providers to sign Business Associate Agreements consistent with HIPAA.
• Government agencies, courts, and law enforcement when required by subpoena, court order, public-health reporting law, mandated abuse or self-harm reporting, or other legal obligation.

We do not share your information with advertising networks, data brokers, or third parties for marketing purposes.

5. Sale or sharing of personal information

We do not sell your personal information. We do not share your personal information for cross-context behavioral advertising. There is no "Do Not Sell or Share My Personal Information" link because we do not engage in either activity.

6. Your California privacy rights

If you are a California resident, you have these rights under the California Consumer Privacy Act and the California Privacy Rights Act:

• Right to know what personal information we have collected about you, the categories of sources, the purposes of collection, and the categories of third parties we have shared it with.
• Right to delete personal information we have collected from you, subject to legal exceptions for healthcare records and other obligations.
• Right to correct inaccurate personal information.
• Right to limit use of Sensitive Personal Information beyond what is necessary to provide the services you have requested.
• Right to opt out of sale or sharing for cross-context behavioral advertising. We do not engage in either, so there is nothing to opt out of, but the right exists.
• Right to non-discrimination for exercising any of these rights.

To exercise any of these rights, contact us at [CONFIRM: privacy contact email or phone]. We will respond within 45 days. We may need to verify your identity before responding to a request.

You may also designate an authorized agent to make a request on your behalf.

7. Healthcare records and HIPAA

If you become a patient, your information becomes part of a healthcare record covered by HIPAA, the California Confidentiality of Medical Information Act (Cal. Civ. Code §56), and other applicable laws. CCPA's general rights are limited to the extent these healthcare laws apply. Your rights regarding healthcare records are described in our Notice of Privacy Practices.

8. Children's privacy

This website is intended for adults seeking care for themselves or a loved one. We do not knowingly collect personal information from children under 16. If you believe a child has submitted information through our form, contact us and we will delete it.

9. Do Not Track

Some web browsers send a "Do Not Track" signal. We do not use third-party tracking, so a Do Not Track signal does not change how we treat your visit. We honor the spirit of Do Not Track by not collecting information beyond what you give us in the form and what is logged for normal site operation.

10. Other parties' tracking

We do not embed third-party trackers, advertising pixels, or social-media beacons on this website. If that changes, this policy will be updated and the change will be announced through the channel listed in section 11.

11. Changes to this policy

We may update this policy. Material changes will be announced by updating the effective date at the top of this page and, where reasonably possible, by direct notice to people who have submitted information to us in the past 12 months.

This policy is published under California's Online Privacy Protection Act (Cal. Bus. & Prof. Code §§22575–22579), the California Consumer Privacy Act (Cal. Civ. Code §§1798.100 et seq.), and the California Privacy Rights Act (effective 2023). It does not create a contract or alter the rights and responsibilities described in our Notice of Privacy Practices for healthcare records.
